As a Care coordinator you will be responsible for:

• Coordinating patient care among staff and organizations
• Providing orientation/training to new caregivers.
• Maintain compliance with caregiver clock-in and clock-out criteria.
• Requesting authorization
• Visiting patients in their homes as needed for care management.
• Building ties with various Service Coordinators.
• Improving satisfaction and caregiver/patient retention.
• Creating patient calendars and quickly allocating workers to relevant schedules.
• Listening to the needs of clients and matching them to the appropriate caregiver(s).
• Plan and execute a schedule that ensures each client has at least one quality assurance every 60 - 90 days.
• Meet with potential clients and family members to discuss their needs and provide solutions in the form of a Care Consultation.
• Perform quality audits
• Resolve caregiver and client grievances and complaints
• Manage Caseload of 100 patients
• Managing last-minute calls outs, finding appropriate coverage.

Qualifications

• 3 Years’ experience in Home Care or Service Coordination or Health Care Industry.
• The ability to work independently in a field position, with a self-starting, upbeat, positive, go-getter attitude.
• A vehicle to get to clients in Montgomery County
• Excellent organizational skills, problem-solving skills, and customer service.
• Team player attitude
• A warm, friendly, compassionate demeanor with elderly clients.
• Demonstrated capability maintaining strict confidentiality
• Proven ability to generate leads and monitor referrals
• Demonstrate excellent oral and written communication skills and the ability to listen effectively
• Comfortable with closing/asking for business
• Well-organized, accurate, and attentive to detail
• Excellent communication, public relations and follow up skills
• Bilingual speakers are preferred
• Driving Licence and vehicle to drive to client’s location is required

Work schedule: Full-time 9 AM - 5 PM EST (occasionally half day on weekend)

When not working from the office you will be traveling to clients' homes in Southern Montgomery Country: including Dresher, Abington, Jenkintown, Horsham, Hatboro, Elkins Park, Oreland, Huntington Valley, and Ft. Washington
We provide our services to the counties: Berks, Bucks, Chester, Delaware, Montgomery, Northampton, Lehigh, Philadelphia.
